the referral process
When a Rochester Medical nurse receives a referral, she will contact the patient to arrange a suitable time for a visit (or the referrer if a joint visit is required).
When with the patient, the nurse will explain about her remit and the purpose of the visit.
The nurse will assess the patient and decide if the use of a continence device would be suitable or, in the event that a device is not appropriate, the nurse will propose an alternative intervention.
The nurse will discuss management options with the patient and, if a suitable device has been identified, she will fit appropriately and teach the patient how to use the product.
Samples will be left with the patient and a follow-up appointment arranged.
On follow-up, patient compliance and product acceptability will be assessed before offering the home delivery service from Script-easy. Should the chosen product prove to be unacceptable or inappropriate the patient will be referred back to the original referring nurse. A further follow-up appointment will be made if necessary.
The nurse will write to the referrer to confirm the outcome of the visit and actions taken.
The Script-easy Customer Care team manage the ongoing monthly supplies for the patient and the Rochester Medical nurse can be accessed again by the patient if needed.
